Community Nurse - Band 5 - Hounslow & Richmond Community Healthcare
Hounslow and Richmond Community Healthcare NHS Trust are looking for an enthusiastic Community Nurse to work with the Locality Nursing Team leadership in the delivery of the locality nursing service in line with the commissioned service specification, standards, activity and outcomes, in the context of multi-disciplinary locality working.
HRCH employs approximately 1,300 people who work in clinics, hospitals, primary care offices, children's centres, municipal government structures, and private homes. Nonetheless, the group provides a wide range of more specialist services to a bigger community in South-West London. HRCH assist individuals in keeping their health in the community, taking control of their own treatment, and preventing hospitalisations by providing the required tools.
This is an excellent chance to collaborate with some of Hounslow & Richmond Community Healthcare's most talented individuals. The following tasks will fall under the purview of the chosen candidates.
Role Responsibilities:
- Assess, plan and deliver nursing care to patients with complex needs to enable them to remain in their own homes, as delegated. Working closely with specialist nurses, allied health and social care professionals to meet the needs of the patients.
- To undertake the assessment, ordering, supply, review and safe use of equipment used to support nursing care within clients homes.
- Refer complex patients to other professionals and services including Community Matrons, to develop multi-disciplinary informed care plans and co-ordinated services.
- Actively participate in case conferences to ensure that all relevant information is exchanged and communicated to inform the care plan and improve the case management of individuals.
- Work closely with GPs to plan care and treatment to reduce risks of hospitals admissions and keep GPs informed of patient’s development.
- Use clinical knowledge to identify changing or deteriorating health conditions and take appropriate actions in partnership with the GP and/or specialist services to treat at home and avoid unnecessary hospital admissions.
Important Requirements:
- NMC PIN
- At least 6 months NHS experience in the last 3 years
- Must be a driver – Able to commute to Hounslow, Wandsworth, Lambeth, Bexley, Bromley, Richmond, Sutton, Merton
- Excellent communication skills, both written and verbal
- A caring, empathetic approach to all patients
- A proactive and professional work ethic
