To work with the Lead ITU pharmacist to provide clinical pharmacy services to the Intensive Care Unit in accordance to the needs of the use r and the required standards of quality and professional practice.
To provide and develop a specialist clinical pharmacy service to the Intensive Care Unit and Critical Care Directorate. Liaise as appropriate with other healthcare professionals within primary care to improve communication across the interface to ensure continuity of care. To Always liaise with consultants to develop and update prescribing policies and procedures.
To train and work as an Independent Prescriber according to Trust policies and within your identified scope of practice, if required. Provide evaluated information for the Drug & Therapeutics Group to assist decision making on the inclusion of new products into the formulary. To attend the Trust Drug and Therapeutics group meetings when necessary.
What we need from you:
· 4 years post-registration hospital experience
. Experience of working as a clinical pharmacist on admissions, general medical and surgical wards
. 1 year pre-registration experience
. Independent prescribing qualification
· Excellent communication skills, both written and verbal
