Accountable to: Respiratory Care Team Leads
The Respiratory Care Team provides a specialist respiratory service to patients with long
term respiratory conditions across the NHS Richmond CCG locality to patients in out-
patient clinics, in their homes and in Pulmonary Rehabilitation classes. The post holder
may be required to assist with the Falls and Cardiac Rehabilitation classes with the
appropriate induction & training.

JOB PURPOSE:
- To carry out skilled professional assessment of patients with long term respiratory conditions, Chronic Obstructive Pulmonary Disease, Bronchiectasis & Pulmonary Fibrosis.
- To work as a member of the Respiratory Care Team (RCT) with responsibility for providing a respiratory specialist service to patients with long-term respiratory conditions.
- To act as a case manager and coordinate care for people with respiratory conditions.
- To ensure provision of a high standard of assessment, intervention and management of people with a respiratory condition including complex multi pathology presentations.
- To provide clinical skills and advice to patients to promote their independence, assisting them to maintain or improve their own physical, psychological and social well-being.
- To build on current initiatives and good practice and provide an equitable evidenced based service including Pulmonary Rehabilitation & Home Oxygen therapy.
- To be a source of expertise, knowledge and support, to other professionals caring for patients in the primary care setting with respiratory long-term conditions as necessary.
- To contribute to the planning and development of services for this group of patients, their families and carers.
- To act as a resource to support families and carers of patients with respiratory conditions.
KEY RESPONSIBILITIES
- To undertake comprehensive, highly specialised respiratory assessment, treatment and management of patients including those with diverse or complex presentations of medical and respiratory pathologies using advanced clinical reasoning skills.
- To provide specialist advice and/or care in both community and local acute of community hospital settings, responding to referral patterns. Make risk assessments of patients’ abilities to stay at home or return home in safety, arranging supportive packages and provision of nursing care as appropriate.
- To encourage involvement of the patient and their carers, in decision making to improve their understanding of their long-term respiratory condition and its management, identifying patient needs and the provision of education, support and monitoring.
- To be an active member of the multi-disciplinary team, working closely with and providing specialist advice to other health and social care professionals on assessment, treatment and care.
- To work in collaboration with colleagues in secondary care to facilitate communication, good practice and a coordinated holistic management, with a focus on admission avoidance and early supportive discharge.
- To offer specialist advice and reports on patient care to other health, social, legal and voluntary sector services.
- To assist the RCT Team Lead in developing a respiratory service that allows people with complex needs to be managed and supported within a primary care setting. This includes ongoing work within Richmond Community Healthcare in Partnership (RCHiP) programme and Richmond GPs.
- To supervise, educate and assess the performance of junior staff of varying disciplines.
- To ensure that the service provided is sensitive to cultural diversity and demonstrates respect and equity.
- To maintain high standards of professional practice and ensure all competencies required for extended practice have been met in accordance with NMC and Trust Guidelines
- To be responsible for maintaining accurate and comprehensive patient treatment records in line with trust policy and NMC code of conduct.
- To risk assess of clients’ abilities to stay at home or return home in safety, arranging supportive packages and providing specialist nursing or physiotherapy care as appropriate.
- Ensure that clinical standards are maintained in line with recent knowledge, evidence, national and local guidelines and best practice.
- Ensure that clinical record keeping is in line with approved standards and that all information and records (written and computerised) are held securely in line with the Caldicott guidelines and the Data Protection Act.
- To ensure that information and reports on patients’ progress and care plan are shared and discussed with appropriate clinicians in the wider team.
- To support the Team Lead in the development and delivery of the RCT service such as admission avoidance and supported discharge.
- To be professionally and legally accountable for all aspects of own work, including management of clinical risk within own patient caseload and managing service capacity and demand.
Important Requirements:
- NMC Registration
- At least 6 months NHS experience in that last 3 years,
- Robust clinical knowledge, strong initiative, and a proactive approach to work
- Excellent communication skills, both written and verbal
- Honesty and integrity, and a caring patient-centred approach
