Hounslow & Richmond Community Healthcare Trust are currently searching for a dedicated Childrens Community Nurse to work as part of their incredible Nursing team Childrens services team in based at Whitton Corner in Twickenham.

HRCH provides community health services for around 523,000 people registered with GPs in the London boroughs of Hounslow and Richmond. The Trust currently employ around 1,300 people, who work across a wide range of health centres, hospitals, GP surgeries, children’s centres, local council facilities and in community settings – including in people’s homes. This helps people across the community to stay well, manage their own health with the right support and avoid stays in hospital.

The post holder will work with the child, the family, members of the Hounslow’s Children’s Paediatric Community Nursing Team in Hounslow and Richmond Community Healthcare NHS Trust (HRCH). They will also work closely with members of the multi-disciplinary team, at primary, secondary and tertiary interface.

Job Purpose:

The Children’s Community Nurse will provide skilled specialist nursing care and advice to empower the child and their family to become competent and confident to carry out the care required at home, in clinics and within special schools. The Children’s Community Nurse will provide training and support to carers working with children on the Paediatric Community Nursing caseload .The Children’s Community Nurse will be involved in the development of staff within the team. This ensures the child’s safety at home and reduces the length of stay in hospital and prevents admission to hospital.

Key Responsibilities:

  • To undertake complex discharge planning and assessment of a child’s needs, ensuring detailed communication with discharging hospital and safe discharge for the child and family.
  • To undertake assessment, planning, implementation and evaluation of care, in negotiation with the child and their family.
  • To undertake assessment of children with continuing healthcare needs.
  • To provide care at home, in school or in the community for children and young people with nursing needs.
  • Ensure that all care planning is adapted to the individual needs of the child. This includes their psychological and psychosocial needs and within continuing care criteria.
  • Assist in undertaking detailed risk assessments when appropriate and ensure that all those involved in caring for the child are aware of appropriate actions to be taken.
  • To demonstrate the specialist and advanced clinical skills required when providing nursing care at home for children.
  • To manage the safe administration of cytotoxic drugs in the community.
  • To provide support and advice to children and their families, in order to support them to self-manage their own condition.
  • Use advanced decision-making skills in order to practice autonomously as required.
  • To achieve, monitor and maintain set standards of nursing care, ensuring safe practice at all times, whilst promoting clinical excellence.
  • Communicate effectively with children, their families, colleagues, and the multidisciplinary team, leading the communication when required.
  • Handle sensitive situations effectively and provide support for children and their families in the giving and receiving of bad news.

Important Requirements:

  • NMC PIN
  • At least 6 months NHS Community Nursing experience in that last 3 years
  • Excellent communication skills, both written and verbal
  • A caring, empathetic approach to all patients
  • A proactive and professional work ethic
