Job Summary: The Patient Pathway Coordinator/Team Leader is the primary contact for all administrative issues related to patients' pathways of care. The role involves proactive management of the patient journey from referral to discharge, ensuring smooth and efficient service delivery. The post holder will work as part of a small team, providing expertise and ensuring high levels of patient and clinician satisfaction.
Key Responsibilities:
- Process and log all referrals in line with Trust Access Policy and speciality SOP.
- Schedule new and follow-up appointments, ensuring efficient use of capacity.
- Coordinate diagnostics and investigations required as part of the patient pathway.
- Manage clinic outcomes, cancellations, and rescheduling in line with waiting time targets.
- Use Trust IT systems to ensure accurate recording of patient information.
- Lead on the validation of the RTT Patient Tracking List (PTL) and manage outpatient and elective patient journeys.
- Ensure timely inter-provider transfers and appropriate paperwork completion.
- Supervise the handling of post, manage consultant diaries, and undertake general typing duties.
- Provide cross-cover within the speciality/POD and attend service development meetings.
- Conduct regular one-to-ones and appraisals, lead team meetings, and deputize for the Admin Manager as required.
- Analyze and extract complex data sets to inform decision-making.
- Ensure compliance with Trust policies on confidentiality, health and safety, risk management, data protection, and infection control.
Qualifications:
- Educated to A-level or NVQ Level 4 in Business Administration/Customer Care or equivalent experience.
- GCSE or equivalent in English and Maths, grades A-C.
- Experience using IT systems and patient data systems, including Microsoft Office.
- Experience scheduling outpatient appointments and/or admissions.
- Experience working in a healthcare setting and handling patient complaints.
Skills:
- Accurate data entry, typing, and checking skills.
- Excellent interpersonal and communication skills.
- Ability to plan, prioritize, and delegate workload.
- Strong IT skills and ability to manage and interpret data.
