The Staff Bank at Kingston Hospital is looking for an Assistant Patient Pathway Co-ordinator to work within the Cardiology Department.

The post holder will be the point of contact for all administrative issues relating to patients’ pathway of care. The post holder will work as part of a multi-disciplinary team and will provide support to the Patient Pathway Co-ordinator (PPC) in the proactive management of the patient pathway from referral to discharge, ensuring the entire pathway is managed smoothly.

 

The post holder will ensure high levels of patient and clinician satisfaction by being an accessible, customer focussed and knowledgeable point of contact. To provide a consistent approach across the Trust, using Standard Operating Procedures (SOPs) to ensure that functions of the role are carried out correctly within given timescales.

 

The post holder will process and log all referrals, including paper and Choose and Book, in line with Trust Access Policy and speciality SOP. The post holder will schedule new and follow up appointments with the most appropriate clinician, ensuring that capacity is proactively and efficiently used and escalate to PPC where this cannot be achieved. The post holder will ensure clinic outcomes are recorded accurately on CRS, including those for offsite clinics. They will use the 18weeks referral to treatment (RTT) to manage all outpatient and elective patient journeys.

- NVQ Level 3 in Business Administration/Customer Care or acquired equivalent experience.
- Experience of using of IT systems and patient data systems
- Experience of scheduling outpatient appointments and/or admissions.
- Experience of scheduling outpatient appointments and/or admissions.
- Good attention to detail.
- Knowledge  of medical terminology appropriate to specialism.
- Excellent communication skills both written and verbal.
