University College London Hospitals NHS Foundation Trust (UCLH), situated in the West End of London, is one of the largest NHS trusts in the United Kingdom and provides first class acute and specialist services to patients.

UCLH is looking for a qualified Band 7 pharmacist that can work full time (37.5 hours p/w) to provide a comprehensive clinical pharmacy service to ward and clinical areas within an allocated specialty, and ensure consistently safe, clinically effective and cost-efficient use of medicines.

Main Duties and Responsibilities:

  • Drug history taking and medicines reconciliation
  • Medicines optimization and antimicrobial stewardship
  • Multidisciplinary team (MDT) meetings and ward rounds
  • Medicines-related advice to patients and other members of the MDT
  • Support medicines-related components of the Trust’s electronic health records system, Epic, at ward level
  • Discharge planning
  • Post-discharge follow-up, where relevant
  • Support clinical governance functions within your specialty
    • Monitor for and report incidents within an allocated specialty
    • Participate in investigations of medication-related incidents
    • Support the development and implementation of guidelines, policies and procedures
    • Conduct and act upon audits and evaluations 

  • Support the education and development of junior staff and pharmacy undergraduates, and participate in team, departmental and multidisciplinary team teaching and learning activities
  • Support service developments, cost-saving or cost-avoidance programmes
  • Support, and participate in, quality improvement projects designed to improve service efficiency and effectiveness, generate your own innovative ideas, and participate in research to generate new evidence
  • Act as a role model, and provide mentorship and direction where needed!
  • Act with professionalism and ensure your ongoing competence through the principles of continuing professional development and lifelong learning
