Band 5 - Community Staff Nurse - Kingston - London

Kingston and Richmond NHS Foundation Trust are hiring a passionate and skilled Community Staff Nurse to join their team.

Kingston and Richmond NHS Foundation Trust was established in 2024 to bring health and care services closer together for local people.

We provide community care in the London borough of Richmond, and we work in partnership with Your Healthcare who provide community care in Kingston. We also provide a full range of acute services at Kingston Hospital, and short-stay rehabilitation at Teddington Memorial Hospital.

We serve a wider population for a range of specialist services. This includes hospital and outpatient services for people in Elmbridge (Surrey), Merton, Wandsworth and Sutton, and community immunisations for most of south London.

Job Purpose:

To provide high quality, skilled nursing care and to assist the Community Nurse Team Leader in the on-going planning, implementation, and evaluation of the holistic needs of the patient/client, family, and/or carer.

Main Duties and Responsibilities:
1. To assist the District Nurse and District Nurse Team Leader in coordinating the work of the nursing team, to ensure effective caseload and care management.

2. To assist in coordinating and supervising the work of the nursing team, for a span of duty in the absence of the District Nurse Team Leader.

3. To undertake first assessments and reviews, planning, implementation, monitoring, and evaluation of individual programmes of care.

4. To deliver a high quality service to clients and carers independently planning, implementing and evaluating programmes of care.

5. To have knowledge across the range of work procedures and practice underpinned by theoretical knowledge or relevant practical experience e.g. wound care, leg ulcer management, palliative care, pain management, medicines management, and IV therapy. 6. To assist and carry out the implementation of local and national initiatives, health promotion, national service frameworks and NICE guidance.

7. Work closely with General Practitioners, practice nurses, care managers and other health professional to ensure agreed standards are met and to ensure efficient team working.

8. To ensure that practice is in line with all infection prevention and control measures as per Trust policy.

9. To act as an advocate for clients, liaising with agencies within and outside the health service.

10. To participate in case conferences, and multi-agency meetings, as required.

11. Assist in profiling the health needs within the caseload taking into account the wider, social, environmental and cultural issues affecting health inequalities.

12. To actively participate in clinical supervision.

13. To identify vulnerable adults and respond in line with Trust safeguarding policy.

14. To provide and offer health promotion and participate in health promotion activities e.g. immunisation & vaccinations.

15. To assist in carrying out the assessment, supply, review and safety of equipment used to support nursing care within clients homes.

16. To assist and contribute to in the implementation of the Clinical Governance Action plan in relation to Community Nursing issues.

17. Ensure that all patient records in all formats i.e. electronic or paper are kept currently in line with trust policy and that all data is entered on to the data base.

  • Requirements:
    Level 1 Registered Nurse- with valid registration with the NMC - RN1 ,RNA
  • Experience or recognised qualification in areas such as:

▪ Health Promotion ▪ Diabetes ▪ Care of the older person ▪ Continence ▪ Palliative care Medical/Surgical Nursing

  • Recent experience of Nursing at Level 5, or evidence of a back to nursing programme
  • Experience of working in the community setting
  • One years worked post registration experience in a relevant setting, e.g. palliative care, care of the older person, orthopaedics, community services
  • Ability to undertake a shift system
  • Good communication skills effective with clients and colleagues
  • Able to work without direct supervision
  • Use a problem solving approach
  • Demonstrates high standard of Infection prevention and control
  • Clinical skills – Administration of medications
